Information on Vaccination

Dear Parents,
On 24 October 2021, the Advanced Level students of 2020 and 2021 will be vaccinated at College premises. Kindly follow the instructions given below.

Instruction for 2020 A/L Batch

Consent form – All the students of 2020 Batch are expected to download the consent form and fill in the information required from the parents. The original form must be brought on the day of vaccination. If anyone is unable to download, a handwritten copy of the same format from the parent is acceptable.
NIC – All the students are expected to bring their National Identity Card. No one will be permitted to enter college premises without the NIC.
Dress Code – Students are expected to dress up neatly with a T-Shirt (preferably with a College T-shirt if available).
Pen – All the students are expected to bring a pen along with them.
Health Issues – If anyone has any medical issues, kindly bring the medical details of the file.
Grama Niladhari Division – All the students are expected to know their Grama Niladhari Division.
Parents’ NIC Numbers – Students are expected to know the NIC numbers of their parents.
If Already Received the Vaccination – If you have received the COVID VACCINATION already, kindly send an email to vpsecretary@trinitycollege.lk mentioning the name of the vaccine and the date and place of receiving the vaccine.

7:15 a.m. – Science Section: Biology, Physical Science and Technology Stream
7:45 a.m. – Arts and Commerce Section
