Booster Dosage only to Students of 2021 O/L Batch, 2022 A/L Batch and 2023 A/L Batch

Dear Parents,

Kindly download and print the document, and fill in the required information, and send the document along with the Vaccination Card on 4 February 2022. 

If you are unable to print the document, kindly prepare a handwritten document on an A4 paper and send it along with the Vaccination Card on 4 February 2022 

Note

Students are expected to bring the following along with them:

1. NIC/ If not available they are expected to know the NIC number of their parents
2. Pen
3. Vaccination Card
4. No Objection Letter signed by parent/ guardian
5. Dress Code: School Uniform
