Kindly download and print the document, and fill in the required information, and send the document along with the Vaccination Card on 4 February 2022.
If you are unable to print the document, kindly prepare a handwritten document on an A4 paper and send it along with the Vaccination Card on 4 February 2022
Note
Students are expected to bring the following along with them:
- NIC/ If not available they are expected to know the NIC number of their parents
- Pen
- Vaccination Card
- No Objection Letter signed by parent/ guardian
- Dress Code: School Uniform
